Medication support at home, safely managed
Nurse-led medication support for adults who need help managing their medicines at home — across South London and Kent.
Why medication support matters
Managing medicines at home can be complex. Multiple medications, varying dosage times, specific instructions, and the risk of missed or double doses all require careful oversight — particularly for people living with dementia, cognitive impairment, or complex health conditions.
Aegys Homecare Services provides structured medication support for clients in their own homes, delivered by trained carers under registered nurse oversight and recorded electronically after every visit.
Levels of medication support
Aegys provides medication support across a range of levels, delivered by trained carers or registered nurses depending on the nature of the medicines involved and the client's needs.
All medication support, regardless of level, is recorded electronically after every visit and overseen by a registered nurse.
Safety and governance
Medication safety is taken seriously at Aegys. The service operates a robust medication governance framework that includes:
- Medication administration recorded electronically after every visit
- Regular medication audits carried out by management
- Any missed doses, discrepancies, or concerns escalated promptly to management
- Medication concerns reported to the GP or prescriber where appropriate
- Staff competency assessed before medication administration is authorised
- All medication support delivered in line with the personalised care plan and prescriber instructions
Families can have confidence that medication is being managed safely, recorded accurately, and overseen by a registered nurse.
Medication support and dementia
For people living with dementia, medication management can be particularly challenging. A person may forget whether they have taken their medication, resist taking it, or be unable to manage packaging and dosing independently.
Aegys carers are trained to support medication sensitively for people living with dementia, using familiar routines, clear communication, and gentle encouragement. Any concerns about capacity, consent, or behaviour around medication are escalated to management and the relevant healthcare professional promptly.
Who medication support is for
Medication support from Aegys is suitable for adults who:
- Take multiple prescribed medicines at different times of day
- Have difficulty remembering whether they have taken their medication
- Struggle to open packaging or manage dosage independently
- Live with dementia, cognitive impairment, or physical disability
- Are managing complex medication regimes following hospital discharge
- Require medication administration by a trained carer
How to get started
A medication support assessment is carried out as part of the free home assessment. The Aegys team will discuss the client's current medicines, any specific instructions from the GP or prescriber, storage arrangements, and the most appropriate level of support. A medication care plan is created and reviewed regularly.
To arrange a free assessment or discuss overnight care options, call 0203 747 4733 or email [email protected]. The team is available seven days a week.
